Who We Are
Smith Integrated Care Services provides experienced professional care management that meets an immense range of needs for seniors and persons with diseases of aging. Our care managers are skilled at planning and implementing plans of care to improve the quality of final years, months, and days. Smith Integrated Care Services works with elders and their loved ones/caregivers to understand needs, hopes, and obstacles in order to create and apply a plan that provides wellness, meaning, and comfort in the sunsets of honorable lives. We specialize in medical care coordination, home care and assisted living, dementias, and end of life care including hospice and palliative care.
Aging With Pride
Smith Integrated Care Services is committed to helping clients age with dignity regardless of their disease progression. Our care managers have experience with low income needs, and what services are available to accommodate for limited funds. Smith Integrated Care Services has a deep understanding of elder care systems, as well as connections to area providers. We work collaboratively with clients, families, and health care teams to make a plan that works for the long-term.

Smith Integrated Care Services is deeply committed to consumer choice. The program's practices and daily interactions support that clients are the authors of their lives. With a background in community based care, including at aging services access points, community mental health clinics, public schools, and in customer service, our care managers have spent her careers helping others to conceptualize and materialize their preferred futures. We have found that private care management lends itself uniquely to opportunities to support clients in taking control of their lives and creating their preferred futures.
