Care Transitions

Care Transitions usually happen abruptly for everyone involved. Smith Integrated Care Services team members love when a client approaches us with a plan to move to assisted living preemptively. However, most moves are precipitated by a cognitive decline or medical event. These are immensely stressful situations, and our care managers have the sensitivity and emotional support skills to help elders through the grief and loss issues they may face. We are also familiar with the available resources and opportunities to meet needs for a wide variety of transitional circumstances.
