Smith Integrated Care Services specializes in assessment, planning, referral, and monitoring of services for elders with new symptoms and early diagnoses. Our care managers are trained in dementia care, as well as in End-of-Life Doula support. They are experienced and committed to supporting elders and families from diagnosis through disease process to promote individual empowerment, dignity, and joy through one's final days.
